Equipment, Medications & Supplies
- Durable Medical Equipment (hospital bed, oxygen, wheelchair, etc.)
- Medications related to the hospice diagnosis
- Medical supplies delivered to the home or facility
Emotional, Spiritual & Family Support
- Medical Social Worker support (counseling, guidance, resources)
- Spiritual Care / Chaplain services (if desired)
- Emotional support for patients and families
- Care planning meetings with families
Bereavement & Ongoing Support
- Grief and bereavement support for families
